What is Cockcroft-Gault Creatinine Clearance?

The Cockcroft-Gault formula estimates creatinine clearance (CrCl) — an approximation of glomerular filtration rate — from age, body weight, sex, and serum creatinine. Published by Cockcroft and Gault in 1976, the equation is CrCl (mL/min) = [(140 − age) × weight (kg)] / (72 × creatinine) × 0.85 if female. Although CKD-EPI is now preferred for CKD staging, Cockcroft-Gault remains the reference equation in the majority of FDA drug labels for renal dose adjustment, including for direct oral anticoagulants and many antimicrobials. Accuracy is reduced in obesity, low muscle mass, and extremes of age.

Frequently asked questions

What is the Cockcroft-Gault formula?

CrCl (mL/min) = ((140 − age) × weight in kg) / (72 × serum creatinine in mg/dL). For females, multiply by 0.85.

When should I use Cockcroft-Gault vs CKD-EPI?

Cockcroft-Gault is still recommended by the FDA for drug dosing in renal impairment. CKD-EPI provides a more accurate eGFR for CKD staging and prognosis.

Should I use actual or ideal body weight?

For underweight patients use actual weight; for overweight or obese patients many clinicians use ideal or adjusted body weight to avoid overestimating clearance.

What units does this calculator use?

Weight in kilograms, serum creatinine in mg/dL, age in years. Output is mL/min.
